The mental status examination (MSE) is the psychiatric equivalent of a physical exam, providing a systematic snapshot of a client's psychological functioning at a specific point in time. Current best-practice guidelines (American Psychiatric Association, 2022; Voss & Das, 2023) emphasize that the MSE should integrate both structured observation and clinical inference across multiple domains, including appearance, behavior, cognition, mood, perception, and thought content. This tool organizes clinician observations into a standardized format to support accurate diagnosis, treatment planning, and longitudinal tracking of symptom change.
Complete this form during or immediately after each clinical session. For each domain, record your direct observations and the client's self-report. Use the Comments column to note clinically significant details, contextual factors, or changes from previous assessments. Rate severity or quality where applicable. This document becomes part of the clinical record and should reflect objective, nonjudgmental language.
