My Thoughts, Feelings, and Actions Log
A CBT Practice Sheet for Young People
Our thoughts, feelings, and actions are all connected. When something happens, the way you think about it changes how you feel, and how you feel changes what you do. By noticing your thoughts, you can learn to choose more helpful ones, which can help you feel better and make better choices.
Think of a situation that bothered you recently. Write down what happened, what you thought, how you felt, and what you did. Then try to come up with a new, more helpful thought and see how it changes your feelings and actions.
