Social anxiety disorder affects approximately 12% of adults at some point in their lives (Kessler et al., 2005; Stein et al., 2023). It involves intense fear or self-consciousness in social situations, driven by worries about negative evaluation. The cognitive-behavioral model of social anxiety (Clark & Wells, 1995; Wong & Rapee, 2016) highlights that understanding your unique triggers, feared outcomes, and avoidance patterns is a critical first step toward change. This worksheet helps you map the specific situations, thoughts, and life consequences that define your experience with social anxiety.

Complete each section thoughtfully. In the first rows, identify the social situations that provoke anxiety for you. In the middle rows, describe the specific worries or feared outcomes you experience. In the final rows, reflect on how social anxiety has shaped your life choices and imagine how life would look without it.
