A biopsychosocial assessment provides clinicians with a holistic understanding of the factors shaping a client's presenting concerns. Grounded in Engel's biopsychosocial model and updated by contemporary integrative frameworks (Borrell-Carrio et al., 2004; Lehman et al., 2017; Wade & Halligan, 2017), this approach recognizes that biological, psychological, relational, and sociocultural dimensions interact to influence mental health. This structured intake form guides the collection of essential information across key domains, supporting accurate case conceptualization, culturally responsive treatment planning, and collaborative goal setting between clinician and client.
Complete each domain as thoroughly as possible during the initial intake session. Use the notes column to record clinically significant details, follow-up questions, or observations. Not all fields may apply to every client; mark any non-applicable areas accordingly.
